Use this list to prepare for an upcoming tax preparation appointment. On the day of your appointment, bring your required documents and all documents that apply to your situation.
- Required: Proof of identification (photo ID)
- Required: Social Security cards for you, your spouse and dependents and/or;
- Required: An Individual Taxpayer Identification Number (ITIN) assignment letter may be
substituted for you, your spouse and your dependents if you do not have a Social Security number - Birth dates for you, your spouse and dependents on the tax return
- Wage and earning statements from all jobs/employers (Form W-2, W-2G, 1099-R, 1099-K, 1099-Misc)
- Savings/bank account interest and dividend income statements (Form1099 INT, 1099 DIV)
- Retirement Income (Form1099 R)
- Social Security Administration (Form1099 SSA)
- Unemployment (Form1099 G)
- Gambling winnings (Form W-2 G)
- Health Insurance Exemption Certificate, if received (Form1095-A, B or C)
- Last year’s tax return if available (tax year 2022)
- Economic Impact Payment (EIP) Notice (Form 1444)
- Tuition form for you or anyone on the return attending college (Form 1098T)
- Interest paid on Student Loans (Form 1098E)
- Total paid to child care provider and name, address, social security number and phone number
or the daycare provider's business tax identifying number. - Bank account routing and account number (or blank check) for fast and secure direct deposit of
your refund. - Identity Protection Personal Identification Number if issued by the IRS (IP PIN)
- If filing married-filing-jointly tax return, both spouses must be present to sign the required forms
